Broken tooth, new scans and updated comparison pics – I have been busy!

I finally made the 1,600km trek down to Melbourne this week and visited Dr John & the team at Freedom Dental.

I chipped the surface off my back right tooth again last week (the same one that fell apart last year if you recall). I safely kept the piece that came off in milk to keep it in good condition but, because I am an airhead sometimes, it stayed in my fridge when I flew down to Melbourne. Good job me!

Dr John decided that the best way to repair it was to dig out the filling that made up the core of the tooth, clean it up and replace the bulk of the tooth as a filling. Some happy gas and two needles later he was able to get the job done with only a few groans of horror from me and did a very neat job of it – it needed almost no adjustment to fit my bite. After we was done and I was left drooling from the complete numbness of the right side of my face he cut that tooth section off my aligner so I could put it back in.

Luckily the rest of my mouth was still intact – no other fillings or work needed and the temporary corner of my front tooth is holding up better than expected. He did earmark one tooth that will need to be checked next time I have an X-ray but it is nothing too concerning at this stage.

We got onto the topic of my Invisalign treatment and Dr John told me exactly what I feared & expected, that even though there are only 2 aligners left we are not quite there yet.

Off to the itero I went for yet another darn rescan. I am so used to the process now that my fear and the pain (from the cold air on my sensitive teeth) seems silly now. Here is a look at that scan – I apologise for the quality but shot it quickly on my phone from the screen beside me

Light at the end of the tunnel!

It has definitely been a while since I posted as I really had to break out of the funk I was in. Moving state was really challenging and not being able to pop into my dentist as needed really bothered me.

I actually lost one of my key attachments and it stalled my whole treatment as we didn’t have a current enough attachment template. Once they were able to order it for me & post it to me in Brisbane I then couldn’t find anyone to apply the attachment for a reasonable price (it’s free with my dentist). The timing ended up working so that I was able to travel down to Melbourne to get it all fixed up.

The timing also worked out to fix my back tooth after I broke it a few weeks ago!! My lower right back tooth is mostly filling after a very large repair a few years ago and a piece of the remaining tooth front cracked off!

I ended up keeping the piece safe and wearing my lower aligner to eat to make sure I didn’t get anything stuck in the cracks. That was super fun. I absolutely hate eating with my aligners in as it feels gross! It also means my aligner was quite stained which was annoying.

After 2 weeks of that fun I finally made it down to Melbourne and my favourite Dentist came in on his day off to help me out!! We talked about my options for repairing that tooth and collectively decided that it will need a full repair and maybe even a crown. If we were to do that now I would need to rescan for Invisalign as the tooth will be a different shape. So to ensure I can finish my treatment we decided to simply stick the piece back on!! It’s more technical than that I know but that’s what we did!! We will just have to see how it goes in the short term – I can’t really floss against the broken edge as its not quite flush but it should last me a few weeks.

WHICH IS ALL I HAVE LEFT OF MY INVISALIGN!!! (In theory!) I am now going to get through the remaining 5 aligners of this series in the 5 weeks until I am in Melbourne next and hopefully everything progresses well enough that we can put a lasting repair on that tooth then scan for my retainer!!!!!

I honestly can’t wait!! I feel like I have been stuck in this journey without end for a while now but the light has definitely appeared at the end! So, wish me luck!

The Invisalign CBF’s

I posted online last week asking, begging even, for blog post ideas after a bad case of writers block. The truth though is that I don’t have writers block, I have a bad case of CBF. Invisalign CBF.

For those who don’t know the term, CBF is the acronym of choice for the lazy condition of “Can’t be f***ed”.

I’m not going to be brutally honest now so if you are looking for happiness & sparkles please look elsewhere. I’m sorry in advance.

I’m in a funk. I know I am still making progress and I am still super glad to be doing this and I’m even really happy with the comparison shots I took recently (I will post them at the end of this drivel). I just can’t be bothered, with any of it. I have stopped putting my aligners back in after meals and I am currently topping out at about 14-16 hours a day which is obviously terrible. There is really no excuse for this as I am in a fantastic new job where I could easily brush my teeth after eating but I have gotten into the habit of snacking and just don’t want to put the darn aligners back in. Instead of my scheduled 5 day aligner changes I am currently rotating them at 10-14 days to make up for the missed hours during the day but I’m reasonably sure my movement is starting to lag behind, I will have to wait and see in another tray or two.

I’m not going to re-read any of this post because then I will just edit and re-write it and I have always promised to be completely honest. sorry again for the drivel – I’m not actually feeling sorry for myself or anything, its just literally CBF.

Moving Interstate Presents Invisalign Challenges

So this was a few weeks of fun!

My husband was offered an amazing opportunity within his company so we moved from Melbourne, Victoria to Brisbane, Queensland (1,660 Kilometers, 1,030 Miles, 18 hours’ drive, 2 hours flight……) you get the idea here – it’s a long way.

In addition to the challenges of packing and moving all our things, leaving our friends and me finding a new job I also had to contend with moving away from my dentist! As Invisalign is arranged & paid for through a specific provider it is not practical to change to a different provider and to do so would mean I need to pay all over again and explain all my personal needs, preferences and idiosyncrasies. I have worked up a great relationship with my dentist and his team and they have a thorough understanding of everything that I need from the fact that I am more comfortable with a higher percentage of oxygen in my medical gas to the specific colour of my attachments.

After a sketchy childhood dental history I, like many people, developed a fear of going to the dentist. To add to this I have always had extremely sensitive teeth that are prone to holes. Prior to getting my Invisalign I had to have some extensive fillings work done including removing and correcting some of the ones I already had from over the years. I will need more work done before I finish my Invisalign and afterwards will at least need a root canal in the tooth that became infected last year. All of this makes me very concerned about finding a new dentist closer to my new home.

So, on to the rookie mistake I made when moving house. As I said in my last post, I moved house immediately after working over sixty hours in four days. Because of this I was not on my A game when everything was being packed – including the Invisalign drawer in my bathroom. Thanks to this stunning oversight I had no access to either my next sets of aligners or my retainer brite! Between the packing of the truck, transit of our belongings on the train, then having to find not only the right box but the specific contents within that box it was another 2 weeks before I got to my new aligners which made it almost four weeks that I had the one set in mouth – with no retainer brite!

I coped alright through the month as I was extra careful about cleaning the aligners in particular. I was also a little more relaxed about wear time, though that was a natural consequence of living out of a suitcase for a few weeks. I managed to get to the end of this with only a light staining to the aligners, it wasn’t noticeable to anyone else (I asked to be safe) but I could see it when the aligners were against a white surface. I have seen many people discuss that their aligners get a foul taste & significant discolouration but I have never had these issues.

So it has been a big few weeks and after all that I need to find a new dentist, or do I? Given that I have all of my friends in Melbourne and my Husband will be travelling there regularly for work so I am lucky that I will be back there in a few weeks. I will need some of my attachments cleaned up and re-applied and will also check in to make sure I am on track. Thankfully this means I have deferred the issue of finding a new dentist for now though I will need to come up with a plan soon no doubt.

I’m off to explore my new hometown this weekend so will hopefully have more tales to tell soon.

Invisalign at Major Events: long days and no toothbrush!

It has been an exceptionally busy couple of weeks so I might just try to do this chronologically in a few blog posts. After receiving my new refinement aligners I was told that I should get through the first few weeks (which were going to be very busy – you’ll see!) then move to 5 day changes to get through the aligners nice and quickly.

I had a major event to work at in the meantime so I wore the first aligner for 10 days then changed to the second a little over a week before the Melbourne Formula One Grand Prix started. I timed these changes very carefully as this is a four day event where I work for around 16 hours a day with minimal facilities.

This year my position was in the middle of a temporary construction entertainment area where there were port-a-loos but no sinks or full bathrooms. I was also on the go almost constantly which leaves little time for eating or brushing teeth. I would brush my teeth & aligners before I left the house in the morning (pre 4am) and wait as long as possible before I ate anything which I would aim for about 7:30 but due to being so busy it usually ended up being around 9:30. I picked at breakfast & tea on the run until around 11:00 (thank goodness for travel mugs!) then ate lunch by about 1:30-2:00. Around 2:00 I used floss and chewing gum (no sinks etc to brush my teeth)  and the Cleanse, freshen, go wipes that I found online to put my aligners back in. I hate the taste of these wipes but rinsed off the aligners with a bottle of water before I put my aligners in – I’m also not convinced that these wipes don’t stain the aligners over time but it’s a situation of the lesser of two evils.

My aligners would then usually come back out again around 4:30 or so to throw down some food. They would then stay out until after I ate a light dinner on the way home around 9-10pm or so. This all adds up to barely 14 hours a day or so which is nowhere near enough for my Invisalign. The plan was always therefore to wear the 5 day aligners for 7 full days prior to the event, the 4 days of the event then 2 days after to be back into the normal 22 hour routine.

Those with Invisalign will know that the routine you form for mealtimes and brushing teeth is extremely important for maintaining the 22 hour a day wear time target. At major events like these it is basically impossible so I made sure I spoke to my dentist in advance and we agreed I should just try really hard to wear them when I can and wer the aligner for longer to ensure everything moves as it should.

I also noticed pretty quickly that I could feel when the aligners had been out for too long – either my teeth would ache, they would not fit together properly (making it uncomfortable to bite down) or I would get ulcers or cuts from the edges of the attachments/buttons that are normally covered up by the aligners. On the Third day I woke up with a very large ulcer on the inside of my lower lip, right where one of my attachments hits when the aligners are out. Thankfully my mouth heals very quickly so I simply rinsed my mouth with warm salt water when I finished brushing my teeth first thing in the morning then also salt rinsed (with cold water – quite unpleasant) a few times throughout the day to keep it clean and it was almost gone by the end of the day.

I knew when I signed on for the Invisalign that it would be tricky to take care of them during my major events so I work on extending the number of days I have the aligner in for to balance the poor wear time.

Just to show that I’m a glutton for punishment I also moved house 2 days after this event, interstate from Melbourne to Brisbane. I will talk about all the fun I had with that (mostly not fun caused by my poor planning) in the next post so that this one isn’t 50 pages long!

Second Refinement Scan and New Comparison Photos!

Hello again!

I have now been for another scan with the itero machine and had the results sent off to Invisalign for a new assessment. They have come back and decided that we will now have another 17 aligners with some new attachments and power ridges (watch this space).

We did a new scan as there was one tooth top & bottom that wasn’t keeping up. The top tooth was that pesky one coming forward with really does have a long way to move overall so we will forgive the slow pace and the bottom one was meant to have an attachment on it to help it rotate & lift but the darn thing just wouldn’t stay!

The invisalign clicheck came back with this:

The scans process also involves taking new photos for the file so I took this opportunity to create some new comparison shots. These are all taken when I first decided to start the treatment in September 2014 and the bottom half of each is the shots taken last week, February 2016.

As well as these I have a comparison with my aligners in but only have a decent shot from a few months in for comparison – better than nothing though! (You can see that the aligner doesn’t quite fit around the tooth shifting forward and the one below it)

I am really happy with the progress so far, especially on those bottom teeth! I have noticed that my teeth are a little more stained now (though my aligners are always very clear) but that seems to be pretty normal and I will simply go for a clean shortly and undertake a whitening treatment when I finish my aligners.

I have had a little trouble with ulcers recently as my changing bite and widening arch have made me frequently bite into my cheeks but these haven’t caused me too much grief and I simply rinse with salt water as needed to keep them clean and they have healed pretty quickly. I also had one sharp edged aligner (Which I haven’t had to worry about for a while now) but this was very easily fixed with a nail file.

Just a quick selfie to finish off this update as I again want to say how awesome my Dentist and his team are! Dr John from Freedom Dental  and all his staff are so helpful and have always treated me amazingly well. No question is too silly, no appointment is too hard to schedule, I never feel rushed and everything is explained fully until I am satisfied I understand everything that is going on. I hear so many horror stories about Dentists & orthodontists making peoples’ Invisalign treatment unpleasant or surprises cropping up during treatment or refinement but I have never had that problem.

Refinement Aligner 10 and the importance of attachments!

WOW! I am always so focused on the tooth coming forward that I forget to look at the rest of the teeth that have come along really well! I am really happy with the transformation thus far and can’t wait for it all to come together soon.

I am now on refinement aligner 10 and can feel lots of progress. I have however lost about 7 attachments which my dentist will be reapplying in a few days. I couldn’t work out why the aligners weren’t quite fitting correctly when I put new ones in each week then had a particularly bad one that kept popping off my top right teeth. A quick call with the dentist explained that the missing buttons were to blame so he ordered another attachment template to reapply them. The problem with the attachments for me is that I have such sensitive teeth that we don’t abrade the teeth first (Rough up the surface) as I really can’t afford to lose any enamel! Not doing this means the attachments don’t ‘stick’ as well as they should and therefore come off easier.

One of the risks of having attachments come off is the pieces themselves occasionally get lost in my mouth. I have bitten into food before and had an attachment crack off then get a shock when I bite down on it! I don’t feel it come off 99% of the time so more than once I have freaked out thinking I have cracked a tooth! A few weeks ago the piece of an attachment got stuck in between my gum and my last bottom tooth – right where the wisdom tooth came out! I could tell there was something there but couldn’t seem to get rid of it. The area became sore them I was able to use a hooked toothpick to pull it out thankfully without going in to the dentist – that’s the only time that has happened to me but it was pretty unpleasant!

I have also noticed big changes with my bite recently which sometimes leads to really sore teeth that feel like they have a hole in them! This is just basically bruising as the teeth are biting down on different surfaces which aren’t used to the pressure. After I move on to the next aligner (or sometimes just wait a few days) this becomes less noticeable or goes away altogether.

That is about it for this update. Hopefully when I get the attachments done again in a few days it will all be fine again for a while. I am moving interstate in 2 months and still have at least 6 months left on my Invisalign so will need to carefully prepare my logistics and plan trips back here carefully!

10 Common Invisalign Myths Debunked!

Something I hear quite often is, “I was thinking of trying Invisalign but…”

I find that people have generally done a little reading, usually 5+ years ago and have some ideas of what Invisalign is and why it “just wouldn’t work” for them. In this post I would like to run through some of the most common misconceptions that I hear and why I think they aren’t true.

1. The “cost is much higher” than traditional braces.
False. I got quotes when I started this and actually found that the Invisalign system came in about even with most of the quotes and even cheaper than some. The beauty of Invisalign is that the pricing is done in bands which means that the number of alingers you need puts you into the relevant price categories which makes it easy to figure out.

2. “Only for easy or cosmetic changes”.
False again. Anyone who has looked at my blog will see that I have has some pretty extensive movement in my teeth and I’m not even done yet!

3. The aligners are “so visible”.
My first response to this is, have you seen train track braces recently?? I cannot count the number of times people have been surprised when I either mention that I have my Invisalign in or go to remove them. I have, at one point, had 6 attachments on my 4 front teeth which are the things that can make the aligners more visible than smooth teeth but even with these they aren’t that visible. If anything, my teeth are a little shinier than normal which only some people notice.

4. “They just seem like such hard work!”
Like anything in life, change takes work. For me, all this took was finding my routine (If you have been reading my blog you would be familiar with this!). Invisalign aligners need to be taken out to eat/drink anything other than cool water. My routine to make this work is to brush when I get up then after each meal and when I say brush I mean floss, brush my teeth and my aligners. I can, when my aligners are out, eat anything I like without cutting out things that traditional braces don’t allow such as popcorn, nuts, gum and soft drinks.

5. “Surely it takes longer than braces?”
Nope! Again, I did my research before I began this process and found that the amount of time for either process was about the same. Because the aligners are changed every two weeks or so the movement is gradual but continuous.

6. “They seem like they would be uncomfortable.”
Well, you’re not exactly wrapping your teeth in fairy floss. I have on occasion had aching teeth in the first day or two of a new aligner but it has never been (independent of external reasons) bad enough for me to need to remove them or want to give up. If I wear them for anywhere around the 22 hour per day required time they are comfortable enough!

7. “They will smell and taste bad from being in your mouth!”
In over 30 sets of aligners I have never had this problem, even when I wore one pair for about 2 months  – and they weren’t even stained. I brush my aligners with toothpaste every time I brush my teeth and use Retainer Brite tablets to soak them for about 10 minutes once each week or so. I think it comes down to two things: drinking plenty of water and good hygiene. I take care of my aligners and am careful not to put my them in my mouth if I haven’t brushed my teeth (or the aligners) properly.

8. “I don’t want to talk with a lisp.”
I did lisp for the first 2-3 days of my Invisalign treatment and occasionally if I have had my aligners out for too long when I put them back in it lasts for the first 20 minutes or so but that is it. The human body, and our mouth in particular, is very good at adapting to necessity so learning to talk with Invisalign is very quick and easy – just avoid the ‘S’ based tongue twisters when you first start out!

9. “I have to get it from an Orthodontist and they are expensive than a Dentist”.
As with any medical procedure, you need to do your research. I looked around and found a Black Diamond provider (this is an indicator of depth of experience with Invisalign), someone close to my work so I can get there and – most importantly – someone I get along well with. In my case this was the excellent Dentist (not Orthodontist) Dr John Hagiliassis of Freedom Dental.
“Freedom Dental is made up of a team of general dentists and we have been designated the very rare status of being an Invisalign Black Diamond provider. This means that we are one of the largest Invisalign providers in Australia.”
Looking at the Freedom Dental website I can clearly see that they have heaps of experience in this area, so much so that Dr John completed the most Invisalign cases in Australia in 2014. The beauty of Invisalign is that the software and treatment modeling is completed by the Align Tech company which means that the Dentist or Orthodontist that you choose is there to monitor the pre-set treatment model using the templates and software provided. Unlike traditional braces there is no need for the ongoing minute changes to wires & tightness throughout the process so I have found no disadvantage in not seeing an Orthodontist. The other benefit is that I can have any cleans, fillings etc done on-site by the same team that I see for my Invisalign appointments which gives me a sense of comfort & security that helps ease my ordinary dental nerves. I cannot highly enough recommend the Dr John and all the team at Freedom Dental as they have made my entire experience seamless and pain free with everything fully explained in advance so there were no surprises.

Last but not least, the most common comment:
10. “Braces are for teenagers, it’s too late for that now.”
Invisalign is for anyone. I have seen cases of people as old as 60+ who are finally in a position to do something for themselves. Whether it is cosmetic or comfort, sometimes it’s as simple as a few stray angles in the front teeth or, like myself, a complete overhaul. There is nothing stopping you except yourself!

Refinement Aligner 4

Just a really quick check in for today. I’m now wearing aligner 4 in my refinement set and it went on pretty easily. There was a little tightness this morning but nothing even close to pain.

These refinement aligners don’t seem to be as different from each other as the original series, they go in easily enough even though I have been changing every 7 days and not exactly keeping up with the 22 hours a day.

About that. Ummm. I’m really struggling getting back into the habit of brushing right after eating. I got really lazy in the gap between needing and getting my refinement aligners and have found it nearly impossible to go back to speed drinking tea and brushing 5 times a day. I have now smacked myself on the back of the hand and set alarms in my phone for just after my usual lunch/dinner times. Let’s see how I go.

Refinement Aligner 2

It has been a while between posts here so let me do a quick recap. I wore the last aligners from my original treatment plan for almost 2 months while I got new scans done for refinement and then had to reschedule my appointment to pick up the new aligners a few time due to working crazy hours. The important part is that we are here now!

I am currently wearing Aligner 2/25 in this refinement series. Both aligners 1 & 2 went in easily enough with minimal soreness though I did have new attachments applied so I had to re-learn how to take them out as they had more catch points. This unfortunately aligned with me having broken all the important nails so it was a little tricky. I didn’t resort to crochet hooks or similar as I have seen some people use but it was definitely a struggle.

I think the hardest part has been getting back into the routine. During the two months in the previous aligner I was very, very bad at keeping my aligners in all the time and would occasionally take them out for breakfast and put them back in after dinner. I accept that this was very silly and I will publicly say DON’T DO IT! Luckily for me there seems to be no lasting damage from it but it has made it very difficult to get back into a stable routine now as I have grown used to snacking through the day and lingering cups of tea. The only positive note from this time is that my Dentist thought I had swapped aligners as they were quite clean. They are the slightest bit discoloured if you hold them against a white background but against new aligners they look fine. I knew I was going to wear them for a while so I was very careful to make sure I didn’t stain them and used RetainerBrite frequently in addition to brushing the aligners every time I brushed my teeth.

I have had the dentist reattach the attachments a few times since I restarted as they are a little tricky in some spots and Dr John now is talking about redoing one of my fillings that is giving me a bit of grief but that can wait until next time.
